Cheer Divisions​

• Mitey-Mite: 5-8 years old

• Junior Pee Wee: 9-11 years old

 

General Requirements -

• Participant must live within the Liberty Public School District Boundaries

• Maintain a 70% scholastic grade point average.

• Be cleared by a medical professional and free from any medical/cognitive condition that would prevent full participation in Pop Warner activities.

 

Required Forms (all forms are due by August 1) -

• 2018 Participant Contract and Addendum

• 2018 Medical/Physical Form - please be sure the doctor signs, dates and includes the address on page 2)

• Copy of certified birth certificate (state seal must be visible; no hospital birth certificates will be accepted). We already have these on file for 2014 participants.

• 2 Copies of complete '17/'18 school year grade card - must include cover and comments section for grades K-5. Grade cards will not be complete until the end of the school year, so be sure to stop by to drop yours off during one of the June sign-ups or attach it to your online registration form.
