Required Forms for Football & Cheer

 

The following paperwork must be turned in by August 1 to allow participation. Paperwork may be mailed to LPW, PO Box 1404, Liberty MO 64069 or given to your team manager on the first day of practice.

• 2017  Participant Contract and Addendum 

• 2017 Medical/Physical Form - please be sure the doctor signs, dates and includes the address on page 2)

• Copy of certified birth certificate (state seal must be visible in the copy. We already have these on file for 2016 participants.

• 2 Copies of complete '16/'17 school year grade card

 

If you are interested in volunteering please fill out the volunteer form below and provide a copy of your driver's license. All volunteers will have a background check performed.
